XII.  Self Inventories

Questions included in this self assessment instrument were developed utilizing information obtained from, The DSM-IV-TR of the American Psychiatric Association (without specific permission) and are in no way meant to serve as a professional assessment tool.


To complete the self test you may print it and complete the answers or, simply get a piece of paper and a pencil.  Number the paper from 1 to 11.  Read each of the questions below and answer them by writing a ( T ) for true or an ( F ) for false beside each number on your sheet of paper.  When complete, read the suggestions listed below for understanding your results.

  8. ____I used to be quite outgoing, but lately, I just do not feel like being around people.
  9. ____I’ve always been a little shy but now I just do not feel like being around other people at all.
  10. ____I’ve been thinking about what it might be like to be dead.
  11. ____I have been thinking about how I might kill myself.

If you answered true to questions (1) (10) and (11) or just (1) and (10), PLEASE contact a mental health professional to schedule an assessment today.                                                                                                            Although you may be feeling many negative emotions and struggling with very troubling thoughts, there are people out there who care and can help you through this difficult time in your life.  Though you may not feel very special, know this, feelings can be deceptive and thoughts are fleeting, but you are a unique, special, and worthy individual.  Only you can take that first step and get help. 

If you answered false to questions (10) and (11) but answered true to question (1) and any one or more of the other questions, you may have mental depression.                                                                                                                      If you feel this is true, and that you are suffering from mental depression, talk to your doctor or set an appointment with a qualified mental health professional for a more comprehensive assessment.  They will be able to assist you with medication management, psychiatric intervention, and/or a referral to a psychotherapist that can assist you in developing special individualized methods for coping with your depression.
