When complete you may read the suggested results below. 1.) ____I take prescription drugs that were not prescribed to me by a doctor. 2.) ____I have used an illegal drug one or more times in the last month. 3.) ____I have been in trouble at work, home, and/or school because of my drug and/or alcohol use. 4.) ____People I know say I drink too much. 5.) ____It seems like I need more and more drugs and/or alcohol too feel drunk or get high. 6.) ____I drove my car when I was under the influence of a drug and/or alcohol. 7.) ____I drink at parties but I also often drink when I’m by myself because it relaxes me. 8.) ____I drink alcohol because it helps me get to sleep. 9.) ____I have pending legal problems that are a result of my alcohol and/or drug use. Results: If you answered true to any one or more of the above questions, do not be angry with yourself or try to ignore the answers you put.
